QUICK REFERENCE #11: When is polysomnography (ie, an overnight sleep study) indicated in the evaluation of insomnia?

That question is addressed in the Practice Parameters developed by the American Academy of Sleep Medicine, updated in 2003. The following recommendations are based on a review of research.

Polysomnography IS INDICATED under any of the following circumstances:

• when a sleep-related breathing disorder or periodic limb movement disorder is suspected
• when initial diagnosis is uncertain
• when behavioral or pharmacologic treatment fails
• when precipitous arousals with violent or injurious behavior occur

Polysomnography IS NOT INDICATED for:

• the routine evaluation of transient or chronic insomnia
• the routine evaluation of insomnia due to psychiatric disorders

Polysomnography IS NOT USEFUL in:

• differentiating dementia-related insomnia from other forms of insomnia (including depression-related insomnia)
• diagnosing insomnia related to fibromyalgia or chronic fatigue syndrome
